STATEMENT OF THE CIVIL PARTNERS OF A CIVIL UNION
«Γνωρίζουμε εμείς, ο/η…………………….. και ο/η…………………………………. πως με τη δήλωσή μας που γίνεται στην παρουσία του Ληξίαρχου και στην παρουσία των προσώπων που βρίσκονται εδώ ως μάρτυρες και με την επιβεβαίωση του γεγονότος αυτού με την υπογραφή μας, δεσμευόμαστε σε συμβίωση και ταυτίζουμε τις τύχες μας, αναλαμβάνοντας την υποχρέωση να φροντίζουμε, να στηρίζουμε και να συμπαραστεκόμαστε ο ένας στον άλλο.»
