In every action the writ of summons and every document required by these Rules to be filed together with the notes of the Judge or of the Presiding Judge of the Court on the hearing of the action or of any application or proceeding in the action shall be fastened up together and shall form the file of proceedings.
For the purposes of this Rule, the notes of the Judge or Presiding Judge of the Court shall be taken on loose paper.
